Payroll Deduction Authorizations
Fall 2008
Graduate assistants and appointed employees participating
in payroll deduction for the term must have a signed copy of their
Payroll Deduction Authorization form on file with Student Accounts
by the end of the business day on
Friday, August 29, 2008
regardless of the date of the first deduction, in order to avoid
a $50.00 late fee.

All information in the request form is required. Within 2 business days,
you will receive a confirmation email of your request. Print, sign and
return the Payroll Deduction Authorization Confirmation form to the Office
of Student Accounts in person, by mail or by fax for your request to be
processed. Your request cannot be processed without your signature
on the Payroll Deduction Authorization Confirmation. Click

Graduate
Assistants
Prerequisites
In order to participate in payroll deduction, graduate assistants should
have completed the employment process and registered for classes.
International graduate assistants should have also consulted with their
department regarding their payment options for international health
insurance.
Payments
Payments may be made in three equal deductions for regular
sessions (fall or spring). The amount of each payment
is determined by taking the total balance owed for the session,
subtracting all charges that will be paid by the graduate assistant's
department, and dividing the remaining charges by the number of deductions
(two or three) to be made.
Graduate assistants who do not have a current billing statement may go
to https://isis.uark.edu
select these links:
Self Service > Student Center
to view their account summary. Graduate assistants who are unsure
of the type (and extent) of charges covered by their assistantship or
who require information regarding payment options for international health
insurance, should contact their department.
Spouses and Dependents
If a graduate assistant is using
payroll deduction to pay a spouse or dependent's student account, both
the employee and student information portions of the Payroll Deduction
Authorization form will need to be completed. A spouse or dependent should
be registered for classes before the graduate assistant submits the completed
and signed form to Students Accounts.

Perkins
Loans
Procedures
Open the Payroll Deduction Authorization form (above) and input
the required information. If
you have questions regarding the form fields, contact the Student
Loans office at (479) 575-3448.
Spouses and Dependents
If an employee is using payroll deduction to pay a spouse or dependent's
Perkins Loan, both the employee and student information portions of the
Payroll Deduction Authorization form will need to be completed.
Deadlines
Graduate assistants and appointed
employees must submit their Payroll Deduction Authorization form to Student
Loans by the 15th of the month prior to the first scheduled deduction.
For instance, if the first deduction is scheduled for January 1st, the
form must be submitted by December 15th. Hourly employees must submit
the form by the 1st of the month prior to the first scheduled deduction.

Changes in financial aid, enrollment, housing, or any other
charge may cause a student account balance to change. It is the
responsibility of the graduate assistant/employee to submit an updated
Payroll Deduction Authorization form to Student Accounts to ensure
that the account balance is zero by 5:00 p.m. on
August 29, 2008.
